You MUST complete this before you scan for us to refer you for your DEXA scan. Without this your appointment may be cancelled or rearranged.

For best results, please ensure you:

• Fast on the day of your testing and avoid stimulants such as caffeine. Normal hydration is fine.
• Avoid exercise for 12 hours prior to your appointment.
